Master’s Application Procedures

2. MSU Graduate Student Application
   Must be done on line or if you print and fill out you must include a check for application fee.
   
   
3. Re-admissions form
   If student graduated from a Master’s program from MSU.
   
   
4. FCE Information Sheet form
   If student is already in a Graduate program at MSU.
   
   
5. Professional Goal Statement
   
   
6. GRE Scores - Dept: Code: 4602/University Code 14-65
   Taken within last 5 years. (Required for Child Development, Family Studies and Community Services Master's Program.
   
   
   
7. GRE Scores - Dept: Code: 4602/University Code 14-65
   Taken within last 5 years (Youth Development Master's Program only if GPA is below a 3.0 in undergrad in the last 2 years.
   
   
8. TOFEL Scores - University Code 14-65
   Are required from international students (taken within the last 2 years).
   
   
9. Official transcripts of all university coursework completed (Request that each university send you the transcripts in a sealed envelope. Enclose sealed, unopened transcripts).
   
   
   
10. Recommendation Form
    Three letters of reference favorable evaluations by previous instructors must also be included with the form.
