   Must be done on line or if you print and fill out you must include a check for application fee.
   
   
3. Re-admissions Form
   If student graduated from a Master’s program from MSU.
   
   
4. FCE Application Form
   Ii student is already in a Graduate program at MSU.
   
   
5. Professional Goal Statement
   
   
6. Resume/Vita
   
   
7. GRE Scores - Dept Code: 4602/University Code: 14-65
   Taken within last 5 years. Required of all applicants apply to the Ph.D. degree program.
   
   
8. TOFEL Scores - University Code: 14-65
   Are required from international students (taken within the last 2 years).
   
   
9. Official transcripts
   All university/college coursework completed (Request that each university/college send you the transcripts in a sealed envelope, unopened).
   
   
10. Recommendation Form
    Three letters of reference favorable evaluations of the applicant by at least three previous professors must also be included with the form.
